I always take media out of the scheme when they fail. By joining you are opting in to receive e-mail. Very odd.Makes me wonder how these two drives are cabled and to what. Root cause could be anything from something out of alignment, dirty heads, EMI, particulates (i.e, cigarette smoke), humidity, temperature, voltage spikes and more.

I would also do a sanity check and make sure you can read some older media. If not, use "bab -load cadbd" from shell prompt. You only put in a cleaning tape when the unit tells you or perhaps 1x/2x a year.
